Mrs B
Mrs B developed dementia and incontinence approximately 6 months before she died. She was prescribed medication which her husband ensured she took.
Mrs B was deteriorating and began to attend a day care centre but became incontinent and started to attend another care centre. She came home for a weekend but collapsed on her return home. Following a hospital visit where she was stabilised, Mrs B went into a nursing home. She began to improve as much as was possible and appeared very happy.
In a matter of days, her husband received a call to say she was very ill and later that day Mrs B sadly died.
A discrepancy was found in her notes in relation to the prescribed medication. Mrs B had been given a drug which was not prescribed to her.
